By using Expressive Arts TheraBeez® Video Telehealth, you agree to have accurate contact information documented in your Account. This information is crucial to your therapist should a mental health crisis or other emergency develop.
You agree that you are responsible for all activities under your Account, and for maintaining the confidentiality of your password and restricting access to your computer so others may not access a Site in violation of this Agreement. In addition, you agree to sign out from your Account at the end of each session if you are using a device that is shared with other people.
HIPAA
As a patient of TheraBeez and Tammi McCurdy, you have the right under the Health Insurance Portability and Accountability Act of 1996, as amended (HIPAA) to access your medical Protected Health Information (PHI). You have the right to receive a copy of your medical record. TheraBeez and Tammi McCurdy will not charge you for providing a copy of your medical record. You can have your medical record sent to yourself or to someone else.
Record Request
Please use this form to tell us where you would like us to send your medical record. This form is intended to be completed by the client, the individual receiving therapy services from a TheraBeez and Tammi McCurdy therapist, or their personal representative (e.g. a parent, legal guardian, etc.).
Google Workspace
TheraBeez and Tammi McCurdy does not fulfill requests for medical records from third parties. If you are a third party, please send the link to this form to the client, or their personal representative, and ask them to complete the form. This form is HIPAA-compliant and your responses are encrypted. TheraBeez and Tammi McCurdy uses Google Workspace and has a business associate agreement with Google LLC. None of the information you enter in this form will be shared with anyone other than TheraBeez and Tammi McCurdy.
Simple Practice
Receipts/billing statements are available in the client portal. You may download receipts by accessing the 'Billing' tab. Receipts reflect client payments and refunds. For information on insurance payments, please obtain an explanation of benefits from the client's insurance carrier.
Appealing Privacy Rights Decisions
You may appeal a decision we have made in connection with your privacy rights request. All appeal requests should be submitted by emailing us at privacy@expressiveartstherabeez.com with the subject line “Privacy Request Appeal.”
Information for Visitors from Outside of the United States
We are committed to complying with this Privacy Policy and the data protection laws that apply to our collection and use of your Personal Information. We are located in the United States, where the laws may be different and, in some cases, less protective than the laws of other countries. By providing us with your Personal Information and using the Services, you acknowledge that your Personal Information will be transferred to and processed in the United States and other countries where we and our vendors operate.
Children’s Privacy
The Services we provide to our Customers are not directed to, nor do we knowingly collect any Personal Information from children under 13. If we learn that we have received any Personal Information directly from a child under the age of 13 without first receiving the child’s parent’s verified consent, we will use that Personal Information only to respond directly to that child (or the parent or legal guardian) to inform the child that he or she cannot use the Services. We will then subsequently delete that child’s Personal Information. If you believe that a child under 13 may have provided us with Personal Information, please contact us atprivacy@simplepractice.com.
Social Media Widgets
Our Services may include social media features, such as the Facebook Like button, and widgets, such as the “Share this” button, or interactive mini-programs that run on our websites (collectively referred to as “Features”). These Features may collect your IP address, which page you are visiting, and may set a cookie to enable the Feature to function properly. The Features are either hosted by a third party or hosted directly on our Services. Your interactions with these Features on a third-party site are governed by the privacy policy of the company providing it.
